IMMUNIZATIONS

PATIENT COUNSELING
Smith Brothers Pharmacy offers Medication Therapy Management (MTM) services to assist you with your medication needs. The program provides face-to-face appointments with a pharmacist to review your medications. The pharmacist will work closely with you, your doctor(s), and your family to understand the issues and concerns you face with your medications and your health—and help you to feel your best by getting the most out of your medications.
Our pharmacist will meet with you and gather your patient history and verify and update records including any prescriptions, over-the-counter medicine, and supplements. We’ll work with you to make sure your medications are working for you, you are avoiding harmful interactions, and identify possible money saving alternatives.
After your initial meeting, the pharmacist will look over your history and answer any questions you may have. If necessary, the pharmacist will consult with your doctor(s) and then follow up with you. You will be given a summary of your personal action plan, including a current medication list.
The program is designed to help you! As your pharmacy, we want to know how you feel about the medications you are taking. We are the liaison between you and your doctor(s), making sure that your medications are working for you!
